What to do when a loved one needs care after hospital discharge

Leaving hospital can feel like a relief, but it can also bring a new set of questions for individuals and families. When someone is not quite ready to return home, it can be difficult to know what to do next or how to make the best decision in a short space of time. Many families and people in need of support tell us they have felt unsure, pressured or overwhelmed during this stage, and it can feel even more frightening for those who may not have family around them.

1. Ask for a clear update from the hospital team

2. Consider short term respite or transitional care
